These forms are preloaded into PayDC.
You can use these forms but are not required to do so.
The prefilled forms will fill out the Basic Patient information on to the form before printing it for the patient to complete.
You can also get to the prefilled forms by going to the top of the page and clicking “Forms”.
Under the forms at the top of the page, there are empty forms that include more of the Intake Forms.
All forms come up in Word so they can be customized but the customized version does not get saved in PayDC.
