Patient Text Message Opt-In Script

This script is provided to ensure that our team obtains clear, consistent, and compliant consent from patients who may wish to receive text message communications from our office. It guides staff in explaining what types of messages we send, informing patients of their rights (including opting out), and documenting the patient’s preferred mobile number. Please use this script during patient check-in, registration, or any time communication preferences are updated.

Staff Member:

“Hi, before we finish, I’d like to ask about your communication preferences. Our office offers text message reminders and updates. These may include appointment reminders, scheduling updates, and other non-urgent information related to your care.”
Staff Member:
“Would you like to opt in to receiving text messages from our office?”
If the patient says yes:
Staff Member:
“Great! To confirm, by opting in you agree to receive text messages from our office. Message and data rates may apply depending on your phone plan, and you can opt out at any time by replying ‘STOP.’”
Staff Member:
“Which mobile number would you like us to use for text messages?”
(Confirm the number and ensure it is documented correctly.)
“Thank you! You’re all set.”
If the patient says no:
Staff Member:
“No problem. We’ll keep your communication set to phone calls and/or emails only. If you change your mind later, just let us know.”
