ICD-10 Update for 2021
ICD-10-CM codes are updated annually and go into effect on October 1st of each year. This year we see 490 new codes, 58 deletions, and 47 revisions. Based on CMS guidelines, PayDC has already implemented the 2021 ICD-10-CM code changes – all the changes loaded and ready to go! Very few of these codes will impact the average chiropractic/physical medicine office (please see the attached article so you can review the changes). However, please follow these steps to ensure continued reimbursement with limited rejected claims:
- Download a summary of all changes, the Tabular Addenda for 2021 from CMS is located at:
- Read the PayDC article “2021 ICD-10 Update” for changes most likely to impact your practice.
- Determine which codes affect your practice and review the clinical concepts behind any new codes. Many of the changes should not significantly change your documentation protocols however new codes should be reviewed.
Remember, the changes go into effect on and after October 1 based on the date of service, not based on the date the claim is sent. Claims for dates of service prior to October 1, 2020 will use the 2020 version regardless of when the claim is sent.

Today October 1, 2019 the 2020 ICD-10 codes went into effect and PayDC has updated the code-set to include code changes. This year there were 273 new codes, 30 revised codes and 21 deleted codes, bringing the total number of codes up to 72,184. Fortunately, these changes are unlikely to have much impact on the average chiropractor/physical medicine provider, however here are a couple of them to be aware of:
- Guideline Section 1.19.c:
- For physeal fractures, assign only the code identifying the type of physeal fracture. Do not assign a separate code to identify the specific bone that is fractured.
- Code revision:
- Z45.42: “Neuropacemaker” is changed to “neurostimulator” and brain, peripheral nerve, and spinal cord are removed.
We have noticed an increase in payers using some established ICD-10 guidelines to deny claims. Please review the following:
- When a code includes an Excludes1 note, that means that the code that follows cannot be used with it. For example, M54.6 Pain the thoracic spine includes an Excludes1 note for M51– disc disorders. These codes should not appear on the same claim, even though some of the M51 codes are for the lumbar spine.
- Pain diagnoses: Diagnosis codes such as M54.2 Cervicalgia, or M54.5 Low back pain are considered short term and more of a symptom than a definitive diagnosis. Consider identifying the cause of the pain and reporting that more definitive diagnosis instead. Reporting a symptom diagnoses does not do much to establish medical necessity and payers are increasingly scrutinizing providers who routinely use these codes.

Yesterday our text message reminder service was briefly interrupted. The last round of messages to go out was the 4:00 PM run on Tuesday (5/8).
What this means is that no text message reminders went out yesterday 5/9/18. Once the issue was resolved early this morning (5/10), we sent out all messages for today’s appointments.
We are reviewing our processes for monitoring text message activity to make sure this doesn’t happen again. We apologize for any inconvenience.

ICD-10 Update for 2021
ICD-10-CM codes are updated annually and go into effect on October 1st of each year. This year we see 490 new codes, 58 deletions, and 47 revisions. Based on CMS guidelines, PayDC has already implemented the 2021 ICD-10-CM code changes – all the changes loaded and ready to go! Very few of these codes will impact the average chiropractic/physical medicine office (please see the attached article so you can review the changes). However, please follow these steps to ensure continued reimbursement with limited rejected claims:
- Download a summary of all changes, the Tabular Addenda for 2021 from CMS is located at:
- Read the PayDC article “2021 ICD-10 Update” for changes most likely to impact your practice.
- Determine which codes affect your practice and review the clinical concepts behind any new codes. Many of the changes should not significantly change your documentation protocols however new codes should be reviewed.
Remember, the changes go into effect on and after October 1 based on the date of service, not based on the date the claim is sent. Claims for dates of service prior to October 1, 2020 will use the 2020 version regardless of when the claim is sent.

Today October 1, 2019 the 2020 ICD-10 codes went into effect and PayDC has updated the code-set to include code changes. This year there were 273 new codes, 30 revised codes and 21 deleted codes, bringing the total number of codes up to 72,184. Fortunately, these changes are unlikely to have much impact on the average chiropractor/physical medicine provider, however here are a couple of them to be aware of:
- Guideline Section 1.19.c:
- For physeal fractures, assign only the code identifying the type of physeal fracture. Do not assign a separate code to identify the specific bone that is fractured.
- Code revision:
- Z45.42: “Neuropacemaker” is changed to “neurostimulator” and brain, peripheral nerve, and spinal cord are removed.
We have noticed an increase in payers using some established ICD-10 guidelines to deny claims. Please review the following:
- When a code includes an Excludes1 note, that means that the code that follows cannot be used with it. For example, M54.6 Pain the thoracic spine includes an Excludes1 note for M51– disc disorders. These codes should not appear on the same claim, even though some of the M51 codes are for the lumbar spine.
- Pain diagnoses: Diagnosis codes such as M54.2 Cervicalgia, or M54.5 Low back pain are considered short term and more of a symptom than a definitive diagnosis. Consider identifying the cause of the pain and reporting that more definitive diagnosis instead. Reporting a symptom diagnoses does not do much to establish medical necessity and payers are increasingly scrutinizing providers who routinely use these codes.

Yesterday our text message reminder service was briefly interrupted. The last round of messages to go out was the 4:00 PM run on Tuesday (5/8).
What this means is that no text message reminders went out yesterday 5/9/18. Once the issue was resolved early this morning (5/10), we sent out all messages for today’s appointments.
